Check the Microchip errata sheet for this part, on the web site.
It is shipped with the low voltage programming (LVP) bit set, and normal
high voltage programming (thro /MCLR) will not override it. RB4 the LVP pin
must be isolated (eg with plastic sleeve) on first programming, with config
set fo LVP off. In theory it can then be normally programmed with the usual
13V on /MCLR and RB4 should default to normal i/o pin.
I found that even after successfully programming the part in this way I
still needed to isolate RB4 subsequently.

No, definitely is not like in practice. I never had problems with HVP
programming on 16F628, if the configuration word is right that one you choose
it for de-activating LVP. So there is no relation between LVP pin and the
first HVP programming. But LVP pin must be tied to GND through a 4k7...10K
after a LVP to avoid pic malfunction.
LVP is an anachronical function for this small PIC. Except programming
feature directly from PC, it kills important hw resources. I dislike it.

'[PIC]: Writing to 24LC64 with warp-13.'

Hello all,
I'm wodering if anyones tried writing an 24LC64 eeprom with
the warp-13.
So far I use the DE command and start at org 0, but it puts nulls
between each character in the eeprom.
What methods are used to write eeprom data into hex files?
Thanks,
Aaron
