When using the debugger in MPLAB SIM, how do you cause a TIMER0 interrupt
to occur?

Please respond to my regular email address as well.  Thanks.

The same way as you would generate a timer 0 interrupt in a real PIC.  The
point of the simulator is to replicate (simulate) the behavior of the PIC as
much as possible.  This gets a little tricky for I/O, but something
completely internal like a timer 0 interrupt should work fine.

No.  You asked on the PIClist, so that's where you'll get responses.
